Sevita Case Coordinator - Galloway NJ - Small caseload! in Galloway, New Jersey

Case Coordinator - Galloway NJ - Small Caseload!

(Looking for a LST/Caregiver with Team Lead experience)

Will work with the Health Services Supervisor

Base pay starts at $16.25 an hour + more depending upon experience/education PLUS No weekends!

($17.25 for 3+ years experience - LST/Caregiver)

$1.00 more an hour for Bachelors Degree

$0.25 cents an hour more for Associates Degree

As Case Coordinator, you will be responsible for providing organizational assistance and program support, including admissions procedures, direct participant care, and coordination of internal and external services.

As Case Coordinator, you will be responsible for coordination of internal and external medical services and overseeing medication with the support from Nursing.

  • Assist in developing individual programs optimizing the participant’s potential to be integrated into the community; ensure the provision of appropriate medical services to designated program participants

  • Assist designated program participants, as needed, to understand their overall medical goals and processes being implemented in the programs

  • Act as a liaison for families and provide them with medical information and resources as requested

  • Review the designated program participant's medical reports for consistency and coordination of medical services; ensure the appropriate medical documentation/recordkeeping of designated program participants

  • Responsible for development, coordination, implementation, and follow-up of medical planning as directed

  • Assist in internal communication to team members regarding case-specific medical issues

  • Attend required meetings, including clinical team meetings and regularly scheduled supervision meetings

  • Review any medical bills for designated program participants; identify any discrepancies and work with insurance to correct

  • Coordinate with the Case Manager relative to program participant’s medical program

  • Organize and facilitate internal medical appointments as requested by management, overseeing timely completion of annual medical appointments

  • Ensure the appropriate documentation (including summaries and tracking information) of designated program participants for internal medical appointments and distribute as required

  • Direct, develop, coordinate, implement, and follow up on healthcare planning as directed by the Program Director/Manager; ensure appropriate medical follow-up for designated program participants

  • Communicate with community resources and persons outside of the program to assure quality program participant healthcare for all assigned program participants

  • Assist in training new staff involved with overseeing medical care as requested

    Qualifications:

  • High School diploma or equivalent; Associate or Bachelor's Degree in Social Service-related field preferred

  • 1+ years of direct or indirect support services to persons with disabilities in a medical and/or rehabilitation setting

  • Strong attention to detail, organizational skills, and the ability to multi-task to meet deadlines

  • Effective communication skills to manage relationships

  • Self-motivated and collaborative; a team player
